PECT, in partnership with Nene Valley Medical Centre, is seeking a Green Social Prescriber to take forward an exciting pilot project. The service has been running for six months, and we are now looking for someone to support patients referred through the practice, and help shape its future.

As the Green Social Prescriber, you will play a vital role in helping patients access local activities that connect them with nature and their community. From gardening and walking groups to volunteering opportunities, you’ll empower people to improve their wellbeing, build confidence, and strengthen social ties. Through your support, encouragement, and creativity, you’ll also help gather evidence on the impact of green social prescribing, contributing to a model of care that could influence how health and social services are delivered in the future.

Employer: PECT

Location: Hybrid role: based partly at Nene Valley Medical Centre, partly at PECT’s offices with some flexible working from home.

Salary (per annum pro rata): £26,000 – £29,000 per annum (pro rata), dependent on experience.

Hours/Week: 3 days per week (22.5 hours) – initial 6-month fixed term contract. Flexible working hours.

Work outside normal hours: Occasional evening and weekend work may be required  


Key responsibilities:

  • Receive referrals from Nene Valley Medical Centre and other partners, and support patients to engage with appropriate green social prescribing opportunities.
  • Hold one-to-one conversations with patients to explore their needs, interests, and barriers to participation.
  • Link patients to local community, volunteering, and nature-based activities that support wellbeing (e.g. walking groups, gardening, conservation, arts in nature).
  • Support the delivery of small group activities where needed, including liaising with local partners and volunteers.
  • Build strong working relationships with practice staff, community groups, and local organisations.
  • Collect and record monitoring information, case studies, and outcomes to evidence the impact of the programme.
  • Promote equality, diversity, and inclusion, ensuring activities are accessible to all.
  • Adhere to safeguarding policies and maintain confidentiality at all times.
  • Represent PECT and the programme positively at community meetings and events.

Person specification

Essential criteria: 

  • Strong interpersonal and listening skills; able to support people with empathy and patience.
  • Experience of working with individuals or groups in a health, community, or wellbeing setting.
  • Excellent communication skills, both written and verbal.
  • Organisational skills, able to manage referrals, appointments, and activity coordination.
  • Flexible and able to adapt to changing needs.
  • IT literate, including use of Microsoft Office and databases.
  • Ability to travel locally as required.

Desirable: 

  • Knowledge of social prescribing and/or community health approaches.
  • Experience in community engagement, volunteering, or environmental projects.
  • Understanding of the role of green spaces and sustainability in supporting health and wellbeing.
  • Knowledge of local community groups and services.

Variation Clause:

This is a description of the job, as it is constituted at the date shown. It is the practice of PECT to periodically to examine job descriptions, update them and ensure that they relate to the job performed, or to incorporate any proposed changes. The appropriate Manager in consultation with the post holder will conduct this procedure.

In these circumstances it will be the aim to reach agreement on reasonable changes, but if agreement is not possible management reserves the right to make changes to your job description following consultation.
